The aim of this systematic review is to gather all the relevant information on the disease in order to improve our knowledge of it, and to be able to make clinical recommendations to improve its diagnosis and treatment.

Statistical analysis was performed with SPSS v. 20.0. Differences between subjects were studied according to the mutation in the CACNA1S or SCN4A genes. Pearson’s chi-square test was used to compare categorical variables. When numerical variables were compared, Student’s t test for independent samples was used. A p value < 0.05 was considered statistically significant in all comparisons.

The clinical characteristics and evolution of the patients were analyzed according to the two most frequent genetic alterations, that of the CACNA1S gene and that of the SCN4A gene (Table III). There were no significant differences in the age at onset or in the time of evolution of the disease, nor in the distribution of sexes or race. On the other hand, there were significant differences in the triggers. Physical exercise was significantly more frequent among CACNA1S subjects, as was a large meal, although this was not statistically significant. In contrast, cold was a more frequent trigger in SCN4A subjects, without reaching statistical significance. Minimum potassium concentration was lower among CACNA1S subjects, and also did not reach statistical significance. Respiratory complication was more frequent among CACNA1S subjects, 31.2% versus 12.5%, without reaching statistical significance. Finally, CACNA1S subjects were treated significantly more with oral acetazolamide than SCN4A subjects. Overall, the response to treatment was comparable between both types of patients.

The study of arterial blood gases and electrolytes carried out on three occasions with an interval of four hours during the day in two patients diagnosed with Parkinson’s disease confirmed that the acid-base and electrolyte balance underwent changes in values. Based on these results and with the purpose of interfering with the so-called metabolic alkalosis-metabolic acidosis-metabolic alkalosis-air respiratory alkalosis (MA-MA-MA-RRA) cycle, the patients were medicated with acetazolamide. The clinical improvement would confirm such hypothesis, hypothesis already plausible as a result of having treated another patient with ammonium chloride (AU).
